Please fill out the form with all your company's information and how your products or services will fit with our Ladies Night In "A Garden Party" Theme and fundraiser.

NOTE: once you apply to become a vendor, one of our volunteer committee members will get in touch with you to let you know if you're approved, then you can pay for your table in the next form. Thank you!
